JACKSON, MI (Nov. 13, 2012) – Great Lakes Caring Home Health and Hospice is pleased to announce the 2012 Award for Excellence honorees for providing quality care to their residents and patients.
From over twenty-two hundred candidates, 122 skilled and assisted living facilities, physicians, and hospitals in Michigan, Ohio, and Indiana were selected for this annual award in recognition of their collaborative efforts to help determine in a timely manner home health- and hospice-appropriate patients in order to provide meaningful and helpful quality care while avoiding unnecessary rehospitalizations.
“It’s important to recognize these partners who are committed to their residents’ and their patients’ best interests,” said Great Lakes Caring Chief Executive Officer William Deary. “This is the fifth year Great Lakes Caring Home Health and Hospice has recognized outstanding partners for their excellence and dedication in providing quality care.”
Each honoree and its dedicated team were chosen from over a 2,200 facilities, physicians and hospitals around the state for three very important reasons: 1) the organization’s belief in the importance of good communication between Great Lakes Caring and the honoree’s staff; 2) for keeping in mind the needs of their patients or residents and the importance of making timely referrals; and 3) for their support and understanding of the importance of collaborative care when a patient is need of home health or hospice care.
This is the fifth year Great Lakes Caring Home Health and hospice has presented this award; no more than a total of 400 facilities or individuals have been chosen to date.
