After an assessment evaluation is completed, a comprehensive plan of care is developed by a nurse case manager in collaboration with your primary care provider and Great Lake Caring Hospice’s own Medical Director. You and your family are also involved in the decision-making process so the plan can be personalized on a daily basis to ensure a superior level of quality care is always delivered.
